In a new Visual Investigation, the Times shows how instigators exploited weaknesses in the Capitol’s defenses to set off a wave of violence that engulfed the building, ending in hundreds of injuries and several deaths. We show the delay to secure Congress that likely cost a rioter her life, the failures by police leaders that allowed the frenzy to take hold and how officers finally secured the building.
